CATEA Consumer Network (CCN)

Welcome to the CATEA Consumer Network (CCN)!

You are invited to participate in the CCN sponsored by the Center for Assistive Technology and Environmental Access (CATEA) at Georgia Tech (http://www.catea.gatech.edu). The purpose of the CCN is to link members with opportunities to participate in research activities. By becoming a CCN member, you will be alerted about opportunities to participate in technology research activities, focus groups, surveys, testing and evaluation of current and new technology products and services. Your input will provide a clear perspective on technology concepts, products and services to make them more usable and accessible. The information you provide will contribute to a better understanding of current and future product and service needs, while providing points of comparison with other information on the same topic. You have been asked to participate because you are a person with a functional limitation and have expressed an interested in improving products and services you use.

If you decide to join the CATEA Consumer Network (CCN), you will be asked to complete the CCN Signup Form and share some information about yourself and your functional abilities. The signup form will be used to enroll you in the CCN. As a member of the CCN, you will receive periodic advisory alerts about opportunities to participate in research activities. Participation is voluntary and you may choose to disregard the signup form and/or exit the web signup form at any time. The information you provide will remain in the CCN unless you remove or update it.

There is minimal potential risk and discomfort from completing the CCN Signup Form. The risks involved are no greater than those involved in daily activities such as completing a subscription for free printed or alternative media materials. You are not likely to benefit in any way from joining the CCN. Indirect benefits include the opportunity to participate in future research studies and the opportunity to improve the products and services used by you, your family and friends. You will receive no payment for completing the CCN Signup Form. There are no costs to you to participate in the CCN. CCN advisory alerts about research studies will include specific compensation information.

Information collected on the CCN Signup Form will be kept confidential. To protect your privacy, your records will be associated with a code number rather than your name. Your records will be kept in a password protected file that is only accessible to study staff for this project and only from within the College of Architecture on the Georgia Tech campus. Your name and any other fact that might point to you will not appear in any publication. Responses to the signup form will only be communicated in aggregate. You should be aware, however, that the web CCN Signup Form (www.catea.gatech.edu/ccn/consent_form.php) is not being run from a "secure" https server of the kind typically used to handle credit card transactions, so there is a small possibility that responses could be viewed by unauthorized third parties (e.g., computer hackers). Also, in general the web page software will log as header lines the IP address of the machine you use to access the page, e.g., 102.403.506.807, but otherwise no other information will be stored unless you explicitly enter it.

If you wish to participate in the CATEA Consumer Network Registry, complete the signup form. Be sure to include your contact information and the best way to reach you. Once we receive your completed signup form, you will begin to receive periodic updates about research opportunities. Your responses will be kept confidential and will not be used for any other purpose than the activities described above.

To make sure that this research is being carried out in the proper way, the Georgia Institute of Technology IRB may review CCN records. The Office of Human Research Protections may also look at the records. Again, your privacy will be protected to the extent allowed by law.

Your participation is strictly voluntary and you are not required to participate at all. You may choose to end your participation at any time. Any new information that may make you change your mind about being a member of the CCN will be given to you. Keep a copy of this letter for your records. You do not waive any of your legal rights by completing the CCN Signup Form.

The CCN Signup Form should take you about 20 to 30 minutes to complete. By completing the CCN Signup Form, you have agreed to take part in the CCN. If you would like to complete the signup form over the phone or on paper, contact a CATEA research representative by calling 1-800-726-9119 (voice or tty) or 404-894-4960. If you have difficulty filling out the form, please send an email to ccn@coa.gatech.edu.

If you are using JAWS (or other screen readers), the signup form works best if you are in "Forms mode". When you are on the first question, press the Enter key to start Forms mode. Use the TAB and Shift-TAB keys to move between questions. Use the right and left arrow to move between answers for the question if it is a multiple choice question.
